I understand that nutrition services (discovery calls and/or nutrition consultations) are not a substitute for professional medical treatment, counselling or therapy. I understand that Joshua Wolrich will be working within his scope as a nutritionist first and foremost, not a doctor. As such he will not attempt to diagnose medical conditions but may help manage them through dietary and wellbeing recommendations.
I understand that the nutritionist may refuse to continue the services if he deems it outside his scope of practice, such as active eating disorders, lack of medical treatment for a serious ongoing condition, or other reasons.
I understand that the nutritionist may make notes during the call/consultations and that these details and conversations will be kept completely confidential and stored securely. A record of names, contact details, and dates and times of appointments will be retained on a password-protected electronic database. Client records are kept for 7 years after termination of sessions, 7 years after the final session, or 7 years after the client turns 18 years of age. Any texts or WhatsApp messages are deleted 6 months after the final session.
I understand that if the nutritionist deems me to be in immediate danger to myself or others, or if I disclose acts of terrorism this confidentiality may need to be broken. Confidentiality may also be broken if client notes are subpoenaed in a court of law. I understand that nutrition professionals are required to engage in regular supervision, as per the Association for Nutrition guidelines, and that the nutritionist may bring aspects of the work to supervision in a manner that maintains confidentiality.
I understand that payment must be made before each session or the session cannot take place. I understand that appointments cancelled/rescheduled with less than 48 hours notice will be charged the full session fee. If more than 48 hours notice is given, you will be refunded the session amount minus the service fee from either the card or PayPal payment (if applicable).
I understand that I am responsible for my own punctuality. Time lost at the start of the session as a result of this will not be made up for at the end of a session; appointments will end on time even if they start late.
I understand that I may pause or stop these consultations at any time, and am entitled to question or refuse any recommendations made. I may contact the nutritionist outside of scheduled sessions but will only receive a reply during working hours. I may also request emergency appointments, which may or may not be fulfilled depending on availability. Any contact via email, text, or WhatsApp is not anonymised, and if I choose to send information via these mediums it is at my own risk.
I may request access to my records at any time. Requests will be fulfilled within 1 month.
I understand that the nutritionist is not liable for any harm due to undisclosed medical conditions, prescribed medications, or other pharmaceutical or psychoactive substances.
Prices for our products are subject to change without notice. We reserve the right at any time to modify or discontinue our merchandise service (or any part or content thereof) without notice at any time. We shall not be liable to you or to any third-party for any modification, price change, suspension or discontinuance of our merchandise service.
We reserve the right to refuse any order you place with us. We may, in our sole discretion, limit or cancel quantities purchased per person, per household or per order. These restrictions may include orders placed by or under the same customer account, the same credit card, and/or orders that use the same billing and/or shipping address. In the event that we make a change to or cancel an order, we may attempt to notify you by contacting the email and/or billing address/phone number provided at the time the order was made. We reserve the right to limit or prohibit orders that, in our sole judgment, appear to be placed by dealers, resellers or distributors.
You agree to provide current, complete and accurate purchase and account information for all purchases made at our store. You agree to promptly update your account and other information, including your email address and credit card numbers and expiration dates, so that we can complete your transactions and contact you as needed.
Merchandise is sourced through Printify. Printify is a print-on-demand company, which means that all products are unique and produced only once ordered. This also means that returns and exchanges are only supported in case of a damaged product or a manufacturing error. In this situation, a choice of either a refund or replacement will be provided. In the case of requesting a refund, the original order may need to be sent back (shipping cost will be reimbursed).
